When Sex Hurts – Dr. Krapf
Dr. Jill Krapf is a gynecologist specializing in vulvovaginal disorders in Tampa, FL
Dr. Krapf is active in research and has published chapters and peer-reviewed articles on vulvodynia, vulvar dermatoses, and hypoactive sexual desire disorder. She is Associate Editor for the Journal of Sexual Medicine, as well as for the textbook Female Sexual Pain Disorders, 2nd Edition.
Love Dr. Krapf? Check out my other episodes #131 – National Lichen Sclerosis Day and #172 – Vulvar Itch
Why write a 2nd edition of the book “When Sex Hurts”?
How common is pain with sex for women?
Often times with pain with sex there is a physical cause, but you can’t always see it.
Pelvic pain and pain with sex is very isolating and people feel very alone. People don’t know how to talk about it.
What happens after the pain is fixed.
What does “all in your head” mean and why people don’t like that.
“Those who perceive their partners as kind and understanding report more intense pain than women whose partners have responses that promote adaptive coping.” – what! What is adaptive coping?
How to communicate your pain to your partner and your doctor.
What if you are a partner of someone with dyspareunia - - “best partner response is having a partner who engages in what researches call facilitative responses – affection and encouragement to cope with the pain in an adaptive way, such as by focusing on pleasure, deep breathing or positive thoughts while experiencing the pain.”
We talk about stress, sex and the gut – things like irritable bowel syndrome related to pain with sex?
The role of birth control pills and pelvic pain and health. In her book, “When Sex Hurts” they talk about Yaz (drospirenone) and pelvic pain – a prospective study of women’s genitals and sexual function prior to starting Yasmin – and then after three months- decreased clitoris, decreased blood flow, decreased labia minora, decreased libido, fewer orgasms and more dyspareunia.
The role of testosterone in pelvic pain – one study showing women with higher androgen levels have less work related neck and shoulder pain then women with low androgen and differing levels of Testosterone may explain some of the gender related differences in pain perception
The difficulty in giving testosterone to women and the problem with pellets.
Recurrent Yeast Infections – Dr. Krapf helps us

Orgasms are good for us, but so often are considered “bonuses”
Don’t forget the “psychosocial” part of sexuality.
Clitoral Adhesions
Genitourinary Syndrome of Menopause
Persistent Genital Arousal Disorder

You Are More Than Just Your Breast Cancer - Hormones and Breast Cancer
Dr. Corinne Menn is a board certified
OBGYN and North American Menopause Society Certified Menopause Practitioner.
She has a special interest in areas often
neglected in women's health: Perimenopause and menopause, the unique needs of
female cancer survivors and those at high risk for breast cancer.
Dr. Menn tells us her story of breast cancer dx at age 28, going through menopause three times and is now a pro-hormone educator.
Dr. Menn discusses taking systemic hormones and her journey and how it has changed her life.
Why are doctors and patients so afraid of hormones?
Doctors are so risk adverse – but let’s talk about the risks of not treating menopause.
The myth that estrogen causes cancer.
How Tamoxifen works.
Breast cancer nomenclature – estrogen positive – this doesn’t mean estrogen caused the cancer – we clarify.
The nuance of being overweight and risk of breast cancer.
The number one killer of women with history of breast cancer is heart disease.
Safety of systemic hormone replacement therapy in breast cancer survivors
Article in Breast Cancer Research and Treatment · January 2022 – Dr Bluming
- "Twenty-five studies of HRT after a breast cancer diagnosis, published between 1980 and 2013, are discussed, as are the 20 reviews of those studies published between 1994 and 2021. Only 1 of the 25 studies, the HABITS trial, demonstrated an increased risk of recurrence, which was limited to local or contralateral, and not distant, recurrence. None of the studies, including HABITS, reported increased breast cancer mortality associated with HRT. Even in the HABITS trial, the absolute increase in the number of women who had a recurrence (localized only) associated with HRT administration was 22. It is on the basis of these 22 patients that HRT, with its demonstrated benefits for so many aspects of women's health, is being denied to millions of breast cancer survivors around the world."
The problem with the HABITS trial.
Why are we scaring women so much about breast cancer?
Who is the right breast cancer survivor to start hormones? A good candidate?
Are we overtreating with aromatase inhibitors?

Feeling Broken In The Bedroom? What You Need To Know About Your Sexual Health - With Dr. Kelly Casperson
This is an interview I did with Lesley McShane for her podcast “Redesigning Midlife” and it was just such a good interview I asked her if I could share it again.
1. Basics: a. Menopause means it’s been one year and one day since your last period. b. The average age of menopause is around 51. and c. for ten years prior to menopause, women go through peri-menopause, where hormones are doing a reverse puberty.
2. A lot of women stop or have pain during penetrative intercourse during this time due to a decrease in natural lubricant and collagen, but this can be easily treated.
3. Even if you don't have menopause symptoms, if you are over the age of approximately 51, you have gone through it.
4. The most sexually satisfied people aren’t young people, it is people in their 50s, 60s, 70s.
5. Great sex is not a passive thing. It’s an active process, just like someone that is physically fit. It has to be prioritized.
6. Use of lube increases your success of orgasm by 70-80%. Silicone based lube is best because you don’t absorb it like you do the water based.
7. There is no “how long” should I use Vaginal Estrogen cream. The use of it is like using sunscreen or wearing a seat belt.
8. You don’t need a “sex drive”. We do things because they are awesome. If you aren’t having awesome sex, don’t wonder why you don’t desire it.
9. We feel entitled to a spontaneous sex drive that has been sold to us by media. And when we don’t achieve that, we end up feeling broken.
10. Putting something in the vagina only leads to female orgasm 30% of the time so special focus need to be paid to the female body. Your largest sex organ is your brain.
11. Don’t sit passively by waiting for spontaneous desire to happen. Get dressed up and go to the party even if you don’t think you want to. Chances are you’ll have a good time once you get there.
12. Control of stress and cortisol levels in our bodies will help us desire more intimacy.
13. If there is a desire mis-match in your relationship, normalize it by talking about it.
14. Communication with our partner about menopausal symptoms and what we are going through is important to our relationship.
15. Get educated on hormone replacement therapy as an option. When women start thinking about how they want to live as they get older and get educated, then they see how beneficial HRT could be for them.
16. Women between the age of 50-60 that are on hormone therapy live 3-4 years longer than women in the same age group not on hormone therapy.
17. For our young adult children that are starting their own sexual lives, the biggest thing they need to know from the start is about consent.
18. They need to be aware of is orgasmic equality.
19.We have to educate our girls that pleasure is for everybody.

Is bigger always better?
The “use it or lose it” myth needs to die.
Vaginal atrophy is a hormonal issue.
Are orgasms good for you?
Can I use vaginal estrogen more than twice a week?
Tips and tricks for vaginal estrogen.
Can you still get BV and yeast infections after menopause?
Painful sex and low libido after a LEEP procedure.
Hi I have a question! I just saw my gynecologist , she has me on vaginal estrogen. I asked if I can go on the estrogen patch she advised me that I don’t need to be on the patch that the tabs are what I need! I’m sorry I should have said explained i had a hysterectomy in my 40’s they left my ovaries a year later I was told I was in full menopause! I’m 60 now and only started vaginal estrogen a year ago due to me complaining about painful intercourse. Should I also be on the estrogen patch.
I’m only 22 and suspecting a stage 3 prolapse . I’m pretty sure I’ve unintentionally caused it due to straining on the toilet, etc. I wanted to know- can I still get pregnant? And can my prolapse heal itself?
Who needs systemic estrogen versus only vaginal estrogen?
The multiple reasons we are undertreating menopause.
I’m a 62 year old guy. Married to a beautiful soul for 40 years. I’d love to get her your book. But, I don’t wanna come off as being “that guy.” Do you have any suggestions?

So I asked ChatGPT to help me write a podcast episode, and I think AI nailed it!
How can iIget my partner to have more sex with me?
1. Communicate openly: Have a conversation with your partner about your needs and desires. Share your feelings and listen to their perspective. It's important to approach this conversation in a non-confrontational and non-judgmental way.
2. Understand their perspective: Your partner may have different needs or desires when it comes to sex. Try to understand their perspective and see if there are any underlying issues that may be affecting their interest in sex.
3. Show affection: Show your partner that you care about them and appreciate them. This can help build a stronger emotional connection and make them more likely to want to be intimate with you.
4. Be spontaneous: Surprise your partner with a romantic gesture or plan a special date night. This can help reignite the spark in your relationship.
5. Look at your relationship: Reflect on your relationship as a whole and see if there are any factors that may be affecting your partner's interest in sex, such as stress, fatigue, or relationship issues.
6. Seek professional help: If you and your partner are unable to resolve the issue on your own, consider seeking the help of a couples therapist or sex therapist. They can provide guidance and support to help improve your relationship.
7. Be patient: Remember that change takes time, and it's important to be patient and supportive of your partner.
It's important to remember that every relationship and every person is different, and what works for one couple may not work for another. Communication and mutual understanding are key to addressing any issues in a relationship, including sexual intimacy.

2. Systemic versus Vaginal Hormones
3. Estrogen Doesn't Cause Cancer
4. We gendered hormones and how this hurts women
5. Natural is a weapon waged to keep us in our place
6. Pellets and compounds are to be avoided if better options are available
7. You never need to stop hormones
8. The FDA will not help us (the two cases of Testosterone and Alzheimer's disease).
(bonus) I hate the word "balanced" when discussing hormones. Pro tip - people are probably just trying to sell you something.

- Vaginal E for everyone who wants it
o Good Rx
Estradiol 0.01% 42 G tube
o Amazon Pharmacy – 28.13

o Peter Attia podcast Episode 42interviewing Estrogen Matters Dr Avrum Bluming and Carol Taveris Phd
https://peterattiamd.com/caroltavris-avrumbluming/
2021 update from the authors
- :the WHI has walked back virtually all of their early alarmist findings. In recent publications, they reported that estrogen does not increase “all cause mortality” or deaths from heart disease and cancer. Actually, they said, it increases longevity, most notably when begun within ten years of the last menstrual period. It is the best preventative for osteoporotic hip fracture. It is safe and effective when applied vaginally for local symptoms. And, in the most striking about-face from their 2003 headlines that HRT “did not have a clinically meaningful effect on health-related quality of life” for women in menopause, they stated in 2019 that “Hormone therapy is the most effective treatment for managing menopausal vasomotor symptoms.”
- Desire mismatch is not a problem
- Orgasmic equality
- Hope is not a game plan
o Wishing your desire was different does nothing – it does worse than nothing
o Resisting reality, resisting what is
- Healthy relationship with your body
o Speak kindly
o Sleep, exercise, food
o Stop shoulding all over your body
- Healthy relationship with sex
o Stop shoulding
o Stop doing it for other people
o Be present
o It is okay to fail, to not feel like it

Meet Sylvie B – a sexological body worker in training!
Many people struggle in their intimate relationships, whether because of pain,
perceived dysfunction, loss of libido, mismatched libido and, most pervasively, shame. Many people have also experienced trauma, low self-esteem, and frustration over the limits to their pleasure.
Somatic sex education is about exploring our sexual challenges through breaking down the
autopilot and the worn out old patterns we rely on for whatever reason, which limit our erotic pleasure and full self-expression.
Good sex more than just the absence of pain.
Why do people come see a sexological body worker?
What is Somatic Sex Education and sexological bodywork?
Not many people are used to being asked how they would like to be touched. Some people get really frustrated because they realize they just don’t know. And that they have never looked too deeply into that. That can bring up feelings of guilt or shame.
Tips for squirting
How do you want to feel during sex?
Tips for tuning in to the very subtle frequency of what people actually desire.
What to do when people feel numb?
How do we emerge from shame and trauma?
How do we set and communicate boundaries as women in our relationships?
The pleasure of the soft cock – what to tell men with ED. What to do.
Let’s talk about this problem – middle age pelvis – menopause versus viagra

Dr. Rubin’s NY Times article!
My book is now on Audible!
And Q&A
· Can you talk about IC?
· What are other options for testosterone? This is the first time I’m hearing that pellets are harmful. I’ve done two rounds of HRT and getting ready for my third. Although to be honest not completely happy with the testosterone pellet as it is elevated at the beginning and begins to drop off after a couple weeks.
· What are your thoughts on getting an ablation to stop my period? I’m 50 and went back on the pill to regulate my period, but it’s suppressing everything else!
· Can you discuss whether surgery before or after menopause is advisable for prolapse?
· We aren’t talking enough about female hormones and their impact on mood. PMS, PMDD etc. It's a roller coaster every month and I have no doubt it has some serious implications for some.
· I am wondering about peri menopause- I am 44 and still on birth control pills- I have been listening a lot about your suggestions to take a low dose estrogen to keep everything in good aging health during menopause, is it ok to do this while still on the pill?

Religion and Sex – Can They Be Bedfellows?
Today we discuss the Muslim culture, sex, and vaginismus with Dr. Sadaf Lodhi, OBGyn and sex coach for Muslim women.
She is host of the Muslim Sex Podcast – we discuss her origin story
Religion can have a profound effect on one’s sex life/sexuality.
Islam is a sex positive religion. When the Muslim countries were colonized they tended to become more puritanical.
Islam discusses sex within the context of marriage.
Many times women experience shame and people don’t talk about women’s pleasure.
Common things Dr. Lodhi sees women coming to her with:
Seeing sex as a duty
Low libido
Beliefs like sex is dirty or sex will be painful
One of her most important things she likes to do is empower women through education.
Women get pressure from their partner and an expectation that it is a duty. And they are failing them and God.
Vaginismus – the involuntary contraction of the muscles surrounding the lower 1/3rd of the vagina.
1/3rd of Muslim culture may struggle with vaginismus.
The role of mindfulness in helping vaginismus and low desire.
“People make putting something in the vagina a big freaking deal” – Dr. C.
The role of PT in treating vaginismus.
There are no FDA approved medications for vaginismus.
Dr. Lodhi shares a huge win with us.
It is not about the number of times of sex a week.
The role of the scholars and the males in helping women feel sex positive.

Meet Glen Lovelace! I love his generosity toward helping men in relationships and his masculine energy on this podcast! He shares his story from desperate (even suicidal) plans to living a life of helping others live their best relationships.
When women learn about their sexuality and get empowered, often they feel stuck because of a spouse who they fear is not open to change. I wanted to bring on someone who literally deals with this as his job. Glen helps men who's wives have been transformed in one way or another, usually by coaching education.
Glenn Lovelace is the wingman for men whose wives have transformed from coaching. He is a certified coach through the life coach school. Glenn has coached over 3,000 clients 1 on 1 since 2008.
His transformation started with bodybuilding after being in a very low place in his life. He decided he needed to prioritize himself to be the best spouse and father he could be.
"It’s okay to be uncomfortable."
How to “gently” change your partner (and remember you can’t change anyone who doesn’t want to be changed).
1) Show them how you have changed
2) Bring in a third party influencer
3) Learn how to speak the same language
4) Always remember you two are on the same team
Roomate Syndrome – just existing together and losing touch why you love and are attracted to each other.
If one person does their work, the marriage can get better.
Your spouse is just there to love.
Can couples start speaking the same language? Glen says yes!
Intimacy not in the bedroom is important!
Wanting your spouse to change for you to be happy is a toxic plan.
Happy Wife Happy Life - it’s a lie and Glen explains how it wasn’t good for him.
Each person is in charge of their own pleasure.
Don’t assume what is needed in a situation - - Ask
How to be a man worth desiring? Start dating and desiring your wife. Approach her how she wants to be approached. Date every week. Keep your commitments.
There are seasons to your sexuality.
Foundations of trust in a relationship
- Security, trust, safety, stability
The trouble with saying “divorce isn’t an option”
Talk about your religion – what does your religion say about sex and marriage and how have you navigated any challenges?

A mindwork episode!
Dr. Ubell is one of the first physician life coaches!
She gained and lost the same 40 lbs at least 10 times since puberty and now helps women become a team with their body and stop obsessing over food using the power of mindwork.
She reached out to me to be on the podcast and at first I didn’t know how to connect the dots besides life coaching. I have a podcast episode called “sex while fat” – we are okay at any size. I was worried she just wanted to talk about losing weight.
So I said Okay if we can talk about how mind work in her arena can also relate to a positive body image and sex life.
Like my older episodes talking about money and sex – these topics are more related than we think because of our thoughts about them.
“I am interested in people stopping hating themselves” – Dr. Ubell
What is diet culture?
The concept of mutual respect between you and your body.
Can weight loss goals exist with body positivity?
Shame is useless.
“I haven’t had success so there must be something wrong with me” Dr. Ubell
Why do we get numb or disconnected from our bodies?
Over eating as a trauma response – sexual assault or childhood trauma – I got big to not feel or too attractive.
Changing one’s identity requires changing our thoughts.
Menopause and weight gain/loss – watch what you believe about this.
Acceptance of our bodies as we get older.
Entitlement thoughts! I feel so seen! I am entitled to my latte!
How to stop invalidating your emotions
Relationship with yourself and thoughts about yourself are important for both weight loss and sexuality.

First I read from the recently published article, "The Anatomy of Female Sexual Arousal: Princess Marie Bonaparte, the Halban-Narjani Procedure, and the Art of Constantin Brancusi." Then I talk about seven different types of vibrators. Then I finish with a tragic case of motorcycle spring meets penis. Enjoy!
1. The OG - Hitachi Magic Wand. external, powerful and marketed as personal massagers
2. 𝗖𝗹𝗮𝘀𝘀𝗶𝗰 𝗽𝗲𝗻𝗲𝘁𝗿𝗮𝘁𝗶𝘃𝗲 𝘃𝗶𝗯𝗿𝗮𝘁𝗼𝗿𝘀 - put curve on them, call them G’Spot’ and raise the price
3. 𝗖𝗹𝗶𝘁𝗼𝗿𝗮𝗹 𝘃𝗶𝗯𝗿𝗮𝘁𝗼𝗿𝘀 - these focus on external clitoral stimulation. Air pulse – Wevibe Melt – Best in class IMO.
4. 𝗗𝘂𝗮𝗹 𝘃𝗶𝗯𝗿𝗮𝘁𝗼𝗿𝘀 𝗮𝗸𝗮 "𝗥𝗮𝗯𝗯𝗶𝘁𝘀" - Work in and out at the same time.
5. 𝗕𝘂𝗹𝗹𝗲𝘁 𝘃𝗶𝗯𝗿𝗮𝘁𝗼𝗿𝘀 - usually a "starter" vibrator for most folks.
6. 𝗖𝗼𝘂𝗽𝗹𝗲𝘀 𝘃𝗶𝗯𝗿𝗮𝘁𝗼𝗿𝘀 - many many different types here. Eve by Dame is one that is "hands free", many strap onto a penis.
7. 𝗔𝗻𝗮𝗹 𝘃𝗶𝗯𝗿𝗮𝘁𝗼𝗿𝘀 - kinda self explanatory.

Vaginal Lasers
How a male can introduce my book to his female partner
Unopposed Estrogen can cause harm
Q&A from listeners:
Genetic testing in 2017 and was positive for Lynch Syndrome. I was just diagnosed with uterine cancer yesterday. 43 years old and total hysterectomy in my very near future. Terrified about trying to get my hormone levels correct post surgery!! Any suggestions or recommendations?
I’m a 38 y/old breast cancer survivor. I’m on estradiol, got all the fancy lubes, and now I’m curious about testosterone… People only seem to talk about estrogen for women, but my libido is nonexistent and penetrative sex is excruciating. Curious if testosterone also takes a hit while on hormone blocking meds post-cancer. I discuss this stuff with the oncologist but it feels trivial compared to the work they did to cure my cancer. However, my PCP won’t touch the hormone topic with me and defers to the oncologist because of my cancer history… thoughts/suggestions?
I’ve gathered so much info from you about the estradiol cream and so much more than I got my gyn to prescribe the cream for me. I’d like to note that I am 59 and I have never had any problems with painful sex or any vaginal issues in the past. I applied the first dose both inside and a bit on the outside areas, it was fine with no noticeable changes. But 4 days later I applied the second dose, this time just inside and within a day or so I felt changes. My clitoral area was sensitive with discomfort and I experienced dryness in the outer labia like never before - not really painful but noticeable discomfort. I have not taken a third dose but I really want to be able to use this so I don’t shrivel up and go away LOL. I also was getting a few hot flashes and they really decreased after the first dose but are back now, although not as much. Is it possible that the discomfort will diminish after a while if I keep taking it? Would it be effective if I only applied it once weekly?
I’m 58 and 2 years in menopause. I use an estrogen patch, progesterone, and a testosterone cream - thanks to you - but it’s not clear to me if my transdermal estrogen will protect the vulva from atrophy. Should I also be using an estrogen cream on my vulva in addition to my HRT regimen? I asked my doctor and she said creams for the vulva are only from lichen sclerosus, but that doesn’t sound right.
